If you have equipment in need of repair and/or a repair evaluation:

  • Unsure of the problem?  Call first to troubleshoot.
  • Download and complete Repair Request Form (below).
  • Ship equipment according to the directions on the form.  Completed Repair Request Form must be included. 
  • Warranty status and repair estimate will be provided for customer’s approval.
  • $120 evaluation fee will be assessed on non-warranty repairs if you decide not to proceed with the repair.

 Equipment shipped without a Repair Form will be returned.
